Dependent Student Verification Worksheet

Independent Student Verification Worksheet

The verification worksheets are required for students selected for verification for the 2010-2011 academic year (Fall 2010, Spring 2011 and Summer 2011).  Your ISIR will indicate if you have been selected for verification.  If so, please download and print a verification worksheet for your respective status and submit it to the financial aid office for processing. 

All University Scholarships (Athletic, Band, Choir, Private Funds, and some external scholarships) require that a student's financial aid file be complete (this includes verification).  If selected for verification, submit your information for verification as soon as you have gathered the required documents (i.e.-completed verification worksheet and tax forms).   

****ALL VERIFICATION PAPERWORK IS CHECKED BEFORE PROCESSING.  ****INCOMPLETE DOCUMENTS WILL BE RETURNED VIA MAIL.

 Private Funds Scholarship Application

The Private Funds Scholarship Application is for the General, Merit and Need Based Aid Scholarhips funded through private donors of the university.  Please download and print an application and submit it for consideration to the financial aid office.  Incomplete scholarship packets will be returned via mail.  Scholarships are competitive and are awarded based on available funding.

SATISFACTORY ACADEMIC PROGRESS APPEAL FORM AND INSTRUCTIONS

APPLICATION DEADLINE:  Monday, August 2, 2010
The Satisfactory Academic Progress (SAP) Appeal must be completed and submitted for the University Appeal Committee to review if you have been placed on Academic Suspension and/or Financial Aid Suspension because you have not met the required satifactory academic progression standards.  Students must meet with the academic advisor of their respective major to complete the Academic Plan.  An incomplete Appeal Packet will be returned to the student.

If you are anticipating on attending MVSU for the 2010 Summer Session(s) and would like to receive aid, you must complete and submit an application. Review of applications will start the week of May 10, 2010. Notification will be sent regarding your award. Applications received after the posted deadline(s) will not be awarded (please see the application for dates).
Transient students, Non-degree seeking and OVER 21 Admit status students are not eligible to receive federal financial aid. Students on financial aid suspension are not eligible to receive federal financial aid for the summer.
