Verification Worksheet 2008-2009

This verification worksheet is for Dependent and Independent Students who have been selected for verfication.  You must attach a  signed copy of your 2007 federal tax form (1040, 1040A/EZ, Social Security Income Statement(s), W-2 statements (non-filers)) for all persons indicated as receiving income on your fafsa application.  Incomplete information will be returned and will cause a delay in processing your financial aid award.  

All University Scholarships (Athletic, Band, Choir, General, Merit Based, Need-Based and some external scholarships) require that a student's financial aid file be complete.  If selected for verification, you must submit your information for verification before your scholarship and federal aid award is made.   

LENDER REQUEST FORM

A lender request form must be submitted to the Office of Student Financial Aid if your lender exited student lending and if you are a new student wanting to borrow funds under the Federal Family Education Loan (FFEL) Program.  THIS IS A LOAN THAT MUST BE PAID BACK!
